RecoveryPlus.health has created a breakthrough home-based cardiac rehab option for patients that are unable to attend a traditional program for transportation or other issues. The program’s targets and parameters are based on the referring physician’s prescribed care plan. Our multidisciplinary staff provides access to dietitians, exercise physiologists, registered nurses, and health coaches to meet each patient’s needs. Using our platform there is no discharge timeframe or session limit. The patient can remain in the rehab program until end of life, patient volition, or discharge by the physician.
