June Simmons is the founding President and CEO of Partners in Care Foundation. Since its founding in 1997, Partners has grown exponentially and made major contributions to the growth of the evidence-based SDOH network model in health services. Partners has received numerous national awards and many major grants to design, test, and disseminate new models of care that yield high-impact results. June serves on the board of NCOA and the EBLC, the Steering Committee for the USAging Business Institute, and leads multiple collaboratives. She is a member of the advisory committee for the NCQA accreditation of HCBS/LTSS. She is the co-chair of the Partnership to Align Social Care.
